Our team supports patients throughout all phases of their healthcare journey. From acute case management, to discharge planning, health coaching and more. We provide local, in-person support to our members during every step of their journey.

We work with our network partners to help identify areas of improvement, implement evidence-based practices, and continuously evaluate processes that lead to better patient outcomes.

We use data from across our network to offer providers a more complete picture of each patient. Our providers receive a comprehensive overview of every patient's medical needs and history, including ED visits, inpatient stays, immunizations, and more at each appointment. Our tools also help providers identify high-risk patients so they can intervene and provide the care patients need sooner.

We care about Yakima County residents and are always looking for opportunities to support our home. In the past we have helped support surveys and outreach programs that help local clinics better understand the communities' needs and available resources. We also supported the community during the COVID-19 pandemic through community testing and vaccination sites.
