What is Care Coordination?
A patient care coordinator is really a patient advocate. A care coordinator helps people navigate and manage their health and wellness by breaking down barriers that one may encounter. Clearing barriers helps patients achieve and succeed in managing their own health goals. These goals are directed and driven by the patient, with the Care Coordinator assisting along the way.
Care management is holistic care. Health issues and concerns not only affect physical health, but also our psychological health. Care management reaches across the spectrum and is not only available to the patient, but also to family members who may need guidance and resources as caregivers.
As a member of the patient’s healthcare team, establishing engagement and a safe trusting relationship is critical. By engaging directly with the patient’s primary care provider, to be an additional resource for access and communication, care coordination can help produce better outcomes in the strive to meet patient goals.
